Our policy is to serve our patients to the best of our knowledge and to correct your foot and ankle problems whenever possible, rather than relieve your pains temporarily. A Podiatrist is a physician and surgeon who specializes in the medical and surgical treatment of the human foot and ankle.
The types of adult and pediatric problems treated at Family Foot Clinic include bunions, hammertoes, deformities, heel pain and heel spurs, ingrown toenails, corns, calluses, warts, trauma, fractures, sports related injuries, skin lesions, nerve problems and neuromas, congenital defects, and pain. Our practice employs leading edge technology including arthroscopy, laser surgery, shock wave therapy for heel pain, the latest in wound healing techniques, and grafting.
The types of service provided include general foot care, reconstructive foot and ankle surgeries, sports medicine, orthotics, bracing and casting.
The foot is often called the mirror of the body. Conditions such as diabetes, gout and arthritis are commonly seen in the foot first. You will be referred to your family doctor or an appropriate specialist should we diagnose a general condition which warrants treatment.
INITIAL OFFICE VISIT
Your initial appointment is usually spent with the doctor conducting a thorough examination and explanation of your condition. It may include clinical examination of the foot and leg, X-rays and lab tests if needed, as well as your medical history. The more comprehensive the examination the more intelligently we can diagnose and treat your condition. Every effort will be made to relieve you of any acute discomfort on this first visit.
MEDICAL RECORDS All clinical information is confidential. It is privileged information and will not be disclosed to any person except upon written permission of the patient or legal guardian.
HOSPITAL AFFILIATIONS
We offer office, outpatient surgery centerand hospital based surgery. We are active members of the medical staffs at St. Agnes Hospital in Fond du Lac and Calumet Medical Center in Chilton. We also hold privileges at Fond du Lac Surgery Center and Aurora Surgery Centers in Fond du Lac and Oshkosh.
FEES AND PAYMENT
Our business office will submit insurance claims for you. Please understand that insurance policies are contracts between you, the subscriber, and the company and do not relieve you of your personal responsibility to this office for debts incurred and balances not covered under your policy.
After we hear from your insurance company on the care you received, we request that your balance owing here be paid within 30 days. In special circumstances, monthly payment arrangements can be made with our Patient Accounts Manager. Billing for both offices is done at our Fond du Lac office. We accept cash, check, and credit card.
Health insurance policies will not usually cover routine foot care, such as the reduction of corns, calluses and nails. We request payment at the time of service for care that will not be covered by your insurance policy (routine foot care, co-pays, deductibles, supplies).
Most health insurance policies, including Medicare, do allow for podiatric treatment of the following conditions:
*Injury (fracture, sprains, lacerations, etc.)
*Corrective surgery (ingrown nails, bunions, hammer toes, boney enlargements that cause corns and calluses, etc.)
*Infections and diseases which affect the foot such as arthritis, bursitis, fungus, warts, diabetic ulcers, etc.
*X-rays
*Labwork
If a referral from your primary care physician is needed in order to have treatment covered here by insurance, it is your responsibility to obtain this. We are participating members of most HMOs, PPOs and managed care programs in our local area. We are Participating Providers in the Medicare program.
